Mercy Release of Records

If you are a current patient of Dr. Scherbel meaning that you have previously seen her at Mercy Clinic or were scheduled to see her, please:

1. Complete the form below. This information will assist us in creating a medical file for you at Scherbel Clinic. Our office will contact you within 1 week to discuss schedule and any other questions you may have.
2. Complete and return the Release of Records form. This form gives Mercy your permission to transfer a copy of your medical records to Scherbel Clinic. This is critical to ensuring continuity of care with Dr. Scherbel at Scherbel Clinic. Your records will be sent to Scherbel Clinic within 2-3 weeks of returning this form to Mercy Clinic.
